Chain of Custody and Sample Submission Form

Purpose. The form that accompanies every batch of samples from the point of sampling to the laboratory, recording who collected the samples, what was collected and who has had custody of them.
1.When to use this form
Complete this form for every batch of samples submitted to the laboratory, whether collected by Veritas field staff or by the client. One form per sampling event or site. The form travels with the samples in a sealed bag inside the cooler and is retained with the job record for seven years.
2.Sampler details
Record the client, project, site, the name of the person who collected the samples and the date and time of collection. Samples collected by Veritas staff must show the field technician's name and the sampling plan reference.
3.Sample details
List every container with its unique sample identifier, matrix, sampling point, date and time, container type, preservative and the tests requested. Use one line per sample. The identifier on the form must match the label on the container exactly.
4.Custody transfers
Each time the samples change hands, the person relinquishing and the person receiving must sign, print their name and record the date and time. Couriers sign as receiving and the laboratory signs on receipt. The laboratory also records the cooler temperature on receipt.
5.Turnaround and reporting
Indicate the turnaround required and the report recipient. Urgent requests must be agreed with client services before submission because they affect scheduling and pricing.
